We need your help to build the Western Australian Chronic Conditions Outcomes Framework.

About this event
Chronic conditions affect thousands of people living in WA and we need a framework to support prevention, early intervention, self-management and treatment.

The Department of Health is developing the framework to improve the lives of people living with, and those at risk of developing, chronic conditions in Western Australia.

This workshop is aimed at people who live and work in Regional and Rural WA to gain their insights and experiences with Chronic Conditions.

People who work in, have lived experience or have an interest in the prevention and management of chronic conditions, are encouraged to attend.

This workshop will involve participants in exploring key elements of the proposed framework. The information collected will be combined with information gained from other workshops and a broad online survey. These combined analysed results will inform the deliberative panel’s recommendations and the development of the framework itself.
